Printable Form

HIPAAtizer can generate a printable PDF from each form submission. These PDFs are useful for archiving, sharing with clients, or internal documentation.

You can access the generated PDF in several ways:

  • Download it from the Inbox in your HIPAAtizer dashboard
  • Send it as an encrypted, password-protected PDF attached to submission notifications for your internal team
  • Let form submitters download the PDF directly on the Success Page

PDF Template Options​

You can choose how the printable PDF is generated:

1. Custom Template (Word or PDF)​

Upload a custom Word or PDF file to be used as the layout. HIPAAtizer will map form field values into designated placeholders in your document.

2. Web Form Layout​

Generate the PDF based on the form’s layout as it appears on the web — including screens, sections, and styling.

3. Structured Table Layout​

HIPAAtizer will generate a PDF with all submitted data in a clean table format, with field names and values.
