A HIPAA-Compliant online form builder allows for the creation of encrypted online forms. These form builders differ from non-compliant online builders by maintaining additional security controls. Online forms created with HIPAA-Compliant builders are encrypted, and are designed to collect, store and transmit Protected Health Information (PHI). Covered Entities must sign Business Associate Agreements (BAA) with HIPAA-Compliant form builder service providers if they plan on collecting, storing or transmitting PHI with the service provider’s forms. Otherwise, they will be in breach of the HIPAA regulations.

Developer accounts can create up to 100 forms, however activating and publishing the forms must be done by a Covered Entity account. The amount of forms that can be active at one time is tied to subscription plans. If you choose the Simple Compliance Gold plan, you can have up to 5 active HIPAA‑Compliant forms at a time and up to 10 active forms with the Gold Plus plan. The Platinum plan allows to have 30 forms.

All data that is created within HIPAAtizer is stored on fully HIPAA‑Compliant servers. HIPAAtizer’s responsibilities are clearly set-out in our Business Associate Agreement that we sign with all Covered Entities. HIPAAtizer, as a Business Associate, is subject to the same stringent privacy and security regulations as a Covered Entity. In addition, emails, forms and other documents originating in HIPAAtizer use FIPS 140-2 compliant protocols to encrypt messages for full end-to-end encryption, and all communication is secured using SSL (TLS 1.2).
